PRE-K REGISTRATION PACKET PICKUP:
Where: Folkston Elementary School
When: May 19, 2020
Time: 9:00 – 12:00
To register, children must be 4 years old on or before September 1st
Parents must return the following items to complete registration:
Required Documents:
- Certified birth certificate
- Child’s social security card
- Georgia DPH form 3300 (includes vision, hearing, dental, and nutrition)
- Georgia DPH form 3231 (Immunization Form)
- Proof of Georgia residency
Accepted: utility bill, vehicle registration, rental agreement, property tax bill, homeowner’s insurance bill, mortgage statement - choose only one
NOT ACCEPTED: driver’s license, cell phone bill
Recommended at this time but will be required before school begins:
- Georgia DPH form 3300 (includes vision, hearing, dental, and nutrition)
- Georgia DPH form 3231 (Immunization Form)
Please contact your child’s doctor or the health department about these forms.
If these documents are not provided, your child will not be eligible. We must have
required documents to complete the application process.
Please return the application with copies of all required documents at one time by Tuesday, May 26th at 12:00 to the front office. Once all 66 spots have been filled, a waiting list will be started. This is based on a first come first serve policy.
